Temperature Excursion Policy

In the event of a complaint/defect with a Blood Product or Service complete the following form and send to
National Quality Assurance Manager, IBTS, NBC, James’s St., Dublin 8 or scan and email to qualityassurance@ibts.ie;

IBTS Complaint Form for Blood Products, Defects, & Testing/Reporting of patient samples

To report Adverse Transfusion Reactions where IBTS has performed compatibility testing use BT - 0311 ( Available from Diagnostics, MRTC) or for RCI use  Request for Transfusion Reaction investigation
